We partner with the physicians in the community and serve as their 24/7 eyes and ears in the home. Our mobile team of providers extend office-based care into the home, relaying valuable clinical information back to the physician community and ensuring continuity of care across all clinical settings. This unique Landmark model is able to fill the gaps for our targeted patient population. Our goal is not only to treat conditions, but provide relief from suffering as well. Its a more compassionate approach to care, and its more focused on the overall well-being of each patient, both physically and mentally.
