Our Nurse Practitioners will participate in family and resident conferences to discuss health issues or concerns including weight loss and wound management. We reduce hospital transfers and hospital admissions because of timely acute and incident follow-up. Practitioners have on-call continuity for nursing staff and residents. Our practitioners are also a great resource for facility wellness functions.
