- For 125 years, Visiting Nurse Association of Central New York, Inc. (VNA) has been a provider of home and community-based care for men, women and children throughout the Central New York region. Through decades of hard work and dedication, countless longstanding relationships with individuals, families and community healthcare providers have been developed. As part of our ongoing effort to better meet the increased and changing needs of those we serve, we look to re-imagine our future as an expert in home health care by focusing on access to care, care transitions, care coordination and the provision of home and community-based services across the entire region. Like the thousands of patients we serve each year, we are proud to say that our home has had a great impact on the system we are today. Located at 1050 West Genesee Street, Syracuse, VNAs operational headquarters will begin a transformation in the coming months that will allow for many efficient and effective changes to our service delivery system. We are Re-Imagining Our Home: So we can bring care to yours. Through a multi-step process, VNAs headquarters will be rebuilt and solidified to: -Serve as a hub for a centralized Continuing Care Call Center;-Provide outpatient physical and occupational therapy to the community with a multi-use gymnasium;-Community room for events up to 150 people;-Purposeful redesign to improve program efficiency and functionality;-Staff lounge/kitchen;-Live in the environment smartly thanks to sustainable energy, thermal efficiency, and green improvements. At the culmination of this project, our organizational headquarters will be a branded facility and community asset. It will help us better serve our mission of bringing care to patients in their own homes as well as, bringing the community into our home.
