Services EHM Provides: -Assistance as Liaison between health care team, legal counsel, insurance carriers and family members -Consulting with health care team (doctors, therapists, rehabilitation specialties) -Client accompaniment to all injury-related doctor and therapy appointments -Assistance with scheduling services and transportation -Comprehensive medical and personal history relative to the client's injury -Current treatment plan status, medical and therapeutic interventions, diagnosis and prognosis -Monitoring of skilled and unskilled care services delivered in the home or on an outpatient basis -In-home visits to evaluate ongoing health care needs, counseling, and support for clients and their families
