• Family HealthCare Network (FHCN) is a nonprofit community-based organization that operates 18 federally qualified health centers (FQHC) throughout Tulare and Kings Counties. FHCN has grown from a small outpatient health center in Porterville serving the farmworker community to a multi-site primary health care network serving 211,983 patients in Tulare and Kings Counties. We are one of the largest providers of care in the country to the underserved population and stand as a national model demonstrating value and effectiveness in reducing health disparities and improving health status in communities in need. Family HealthCare Network has earned a reputation for excellence nationally and statewide. We are dually recognized by The Joint Commissions Gold Seal of Approval® for accreditation by demonstrating compliance with The Joint Commissions high standards for health care quality and safety and certified with The Joint Commissions Gold Seal of Approval® as a Primary Care Medical Home. Only 118 health care providers across the nation and 8 health care providers in the State of California have received dual recognition by both NCQA and the Joint Commission (TJC) and only four other community health centers in California have received the Level-3 PCMH quality ranking.The National Committee for Quality Assurance (NCQA) has also recognized FHCN as a Level-Three Patient-Centered Medical Health Home, and is one of only four other community health centers in the State of California to receive this prestigious recognition. This honor recognizes that our health care teams work in partnership with patients to provide care thats coordinated, effective and timely. For 40 years, Family HealthCare Network has advanced the development of community-based primary health care delivery systems to address the health care needs of underserved and vulnerable populations.
