- Transforming healthcare for the community with the community. The Trenton Health Team (THT) is a unique public-private partnership, launched in 2006 in response to the need for improving the health status of the city and increasing access to health services. A collaboration among the City of Trentons Department of Health and Human Services, Capital Health Systems, St. Francis Medical Center, Henry J. Austin Health Center (Trentons only Federally Qualified Health Center), as well as over 60 organizational partners coming together through our Community Advisory Board (CAB), THT works to design and implement changes to improve the health care delivery system in Trenton through tightly-integrated care coordination and management, as well as improve population health through various community-facing programs.THT also operates the Trenton Health Information Exchange (HIE), which integrates patient record data in real-time across all participating health systems, to better manage patient treatment decisions and strategies. Today, more than 600 clinical users can access millions of records for more than 300,000 patients.As part of the New Jersey Medicaid Accountable Care Organization (ACO) Demonstration project, THT-ACO aims to improve health outcomes, lower healthcare costs, and improve the patients experience of receiving care. New Jersey is one of only 10 states that have active Medicaid ACOs.THTs approach addresses the whole person not just a foot, a heart, or a kidney problem but everything about that persons life, including homelessness, mental illness, fear of crime, and lack of access to fresh, healthy food.
