We dont focus on the finality of death; we focus on the practical side of living.Transitions Hospice team members are always available after hours. We will see our patients as often as needed regardless of their location, the day, or time. Hospice should never seem like it is one size fit all; we individually tailor our hospice care to fit each patient's needs and wishes. We provide: - 24 hour nursing - 24 hour Social Work & Counseling Support - Volunteer Services - Physicians and Nurse Practitioners that are Hospice and Palliative Care CertifiedHospice is a Medicare/Medicaid benefit. This benefit offers four levels of care to accommodate varying needs of individuals. In most cases, Medicare/Medicaid covers 100% of hospice services. Many private insurance providers also cover hospice care. - Routine Care - Respite Care - General Inpatient Care (GIP) - Continuous Care (CC)We are experts in the art of living.
