Our two campuses, The Meadows located in Bellevue and Heartland in Donelson, are conveniently located and serve senior residents in the greater Nashville, Tennessee area.The Meadows has a senior independent housing community with one-level homes, emergency call system, social and recreational opportunities for residents. Lakeshore Meadows consists of Residential Home for the Aged (assisted level living); a Skilled Nursing Facility (SNF), Intermediate, and Long-term care. Therapy is provided to residents needing Physical, Occupational and Speech therapy.The Meadows also has a secure Memory Care unit for those with dementia, Alzheimer's and other related conditions. Heartland also has a senior independent housing community with one-level homes, emergency call system, and social and recreational opportunities for residents. is a Level I Medicaid certified facility and serves those needing Intermediate and Long-term care. Physical, Occupational and Speech therapy services are available.Lakeshore is a non-profit organization, established in 1952 as a ministry of members of Nashville area Churches of Christ.
