• Land of Lincoln Mutual Health Insurance Company (LLH) was placed in liquidation on September 29, 2016 pursuant to an Agreed Order of Liquidation With A Finding of Insolvency (the Liquidation Order) entered by the Circuit Court of Cook County, Illinois (the Supervising Court). The Liquidation Order further provided that all in-force LLH policies be terminated, as follows:All policies and/or certificates of insurance heretofore issued by LLH are cancelled effective 12:01 a.m., Central Standard Time, on October 1, 2016 except for those policies and/or certificates of insurance which comply with the qualifications for coverage under the Illinois Life and Health Insurance Guaranty Association Law, 215 ILCS 5/531.01.Please be aware that:- All LLH claims submitted for covered services incurred through September 30, 2016 will continue to be processed and paid.- Contracted hospitals and providers may be contractually prohibited from seeking collection from insureds of LLH for covered services, except for applicable copayments, coinsurance, and deductibles.
