Our co-founders, Phac Le Tuan and Dr. Michael Miyamoto, make up a world-class team in technology and medicine. Our company was created out of a need for physicians to better manage their patients with cardiovascular medical problems, such as atrial fibrillation and heightened risk for stroke.Patients with these medical conditions, and others, are often prescribed anticoagulant medications. Warfarin is the most common of these drugs. The management of warfarin is difficult and has remained largely unchanged for the past 60 years. Too low or too high of a warfarin dose can lead to either blood clotting or bleeding complications. The effect of the drug must stay within a narrow therapeutic range, but in practice, it may change from day to day, and even fluctuate due to what the patient has eaten that day. The warfarin dose has to be adjusted accordingly, and doses may vary widely, from patient to patient.All of these constraints make it very cumbersome for patients on anticoagulant therapy and increases costs to providers. Management of this therapy demands a degree of personalization that currently just does not exist.Our solution includes a HIPAA-compliant, cloud-based mobile application, called Easyfier, for patients or their caregivers, on smartphones and tablets, with the goal to make life on warfarin easier and more manageable. Easyfier can be optionally integrated with EHR systems to enable providers to automatically send new dosing instructions through a fully actionable push notification to the patient's mobile device.It is time for uncleCare to disrupt the management of patients with heart disease and enable a seamless process between patients, anticoagulation clinics, physicians, and caregivers.
