• Elder Outreach is a non-profit organization established over a decade ago to offer safe, dignified, and home-like alternatives to older adults who can no longer live at home. While embracing our core mission and high standards of care for residents, each Elder Outreach nursing and rehabilitation facility operates like a family-owned business. Our community-based staff and facility sizes allow us to develop individual relationships with residents to provide personalized care based on each residents needs and desires.
