The goal of our CarePlus Program is to make patient time spent with caregivers richer, to create better daily experiences for patients, and to provide memories and support for their families. We put the patient at the center of care through the following: CogS Kit: helps a patients memory, senses and cognitive function. Project Pensive: Family Focused caregivers can record and share precious patient memories with the patient/guardian approved list of family and friends. We work to provide peace of mind to the patient and family through the following: Online Family Room - We communicate regularly about the care plan with the patients permission. The Family Room is accessible through a computer or mobile device 24/7. Medical Care Team Coordination- We work with the patients current medical care team regarding discharge instructions, provide transportation and coordination of care and care reporting. Skilled Care - We actively coordinate the necessary level of skilled nursing care need with our partners. Home Cleaning Services Coordination - We arrange for housekeeping services to provide a deep clean, or to get a house ready for sale. Bill Pay Services Coordination - We work with companies that are insured and trustworthy to ensure bills are paid on time. Family Focused Home Health Care is a licensed home health agency in Florida by the Agency for Health Care Administration, HHA License #299994370. We are Family Owned and Operated in Jacksonville.
