- PCCM is designed to help Physicians help Patients live healthier by providing Patients a motivating connection with their Physician between office visits and providing Physicians a tool to know when changing influencing factors are interfering with their Patients ability and willingness to follow their prescriptions. PCCM provides Physicians with an off-site, dedicated clinical staff employed by PCCM. Under the Medicare regulations for provision of Chronic Care Management Services, this staff qualifies as working incident to the Physician. The 20-minutes of non- face to face care coordination services they provide monthly for the Physicians Patients is billable to Medicare by the Physician under CPT 99490.
